Connect for Health (C4H) manifests Lifespan’s mission, Delivering health with care, by not only providing medical treatment when patients are sick but also addressing the social determinants of poor health. A lack of basic resources is often at the root of poor health so C4H screens patients for health-related social needs like healthy food, safe housing, furniture, and utility assistance and provides navigation support to remedy those needs. In so doing, C4H uniquely bridges medical, community and social services to help patients achieve a complete state of health, and advance the collective goal of health equity. A program of the Lifespan Community Health Institute, C4H currently operates at Rhode Island Hospital’s Center for Primary Care and Medicine Pediatrics Clinics, as well as Hasbro Children’s Hospital’s Pediatric Primary Care and Adolescent Healthcare Clinics.
